- Review patient profiles and lab values using electronic clinical documentation systems to ensure the therapeutic appropriateness of care.
- Collaborate virtually with physicians, care managers, and data analysts to align treatment regimens with current evidence-based guidelines.
Patient Engagement & Education
- Lead virtual consultations with patients to promote medication adherence and disease awareness.
- Utilize telehealth platforms to effectively communicate lab trends, titration schedules, or adverse drug reactions in a patient-friendly and tech-savvy manner.
- Design interactive education materials and digital resources for patients navigating long-term therapies.
Technology-Driven Monitoring
