Key Responsibilities

• Review patient profiles and lab values using electronic clinical documentation systems to ensure the therapeutic appropriateness of care.
• Collaborate virtually with physicians, care managers, and data analysts to align treatment regimens with current evidence-based guidelines.

Patient Engagement & Education

• Lead virtual consultations with patients to promote medication adherence and disease awareness.
• Utilize telehealth platforms to effectively communicate lab trends, titration schedules, or adverse drug reactions in a patient-friendly and tech-savvy manner.
• Design interactive education materials and digital resources for patients navigating long-term therapies.

Technology-Driven Monitoring

• Monitor patient health outcomes using predictive analytics dashboards and real-time tracking platforms.
• Generate insights through integration with remote patient monitoring (RPM) tools and wearable device data.
• Contribute to the development of digital alerts and AI-powered clinical decision support systems to identify intervention opportunities.

Quality Assurance & Compliance

• Ensure adherence to healthcare privacy and quality assurance protocols, including national compliance frameworks and accreditation bodies, through secure remote practices.

Education & Certification

Professional Experience

• 3+ years of experience in chronic disease management or ambulatory care pharmacy
